Dr. Matthew Forgit
Dr. Forgit has been a provider at the Crookston Eye Clinic since August of 2004 and at the Fosston Eye Clinic since August of 2012. He grew up on a farm near Fertile, MN. He did his undergraduate work at Concordia College in Moorhead, MN, and attended Pacific University College of Optometry. Clinical rotations at a busy multi-center group practice, a VA hospital, and remote Alaskan villages with the Indian Health Service provided specific experience in ocular disease, ocular trauma, and surgery co-management. Today he enjoys all the variety associated with full-scope primary eye care.
Dr. Forgit is a member of the American and Minnesota Optometric Associations. Aside from eye care, he is a trustee on Riverview Health’s Board of Directors.
Dr. Forgit and his wife have three children and reside in rural Crookston. When not involved in family or work activities, he enjoys training, hunting, and field-trialing bird dogs.
Dr. Kari Miller
Dr. Kari Miller moved to Crookston, MN, in July 1991, after completing her Doctor of Optometry degree and graduating from Southern California College of Optometry, in Fullerton, CA. Kari and her husband, Mark, are originally from Carrington, ND.
Dr. Miller began her optometric career in August 1991 at the Crookston Eye Clinic purchasing the clinic in January 1996. She worked at the clinic on a full-time basis providing comprehensive eye health and family vision care for many years. It has been her distinct pleasure over the years to work with a very talented group of optometrists and staff. Her passion within the profession ofoptometry is providing patient care and developing lasting relationships with her patients.
Dr. Miller continues to work on a part-time basis at the Crookston Eye Clinic after completing the sale of the practice to Dr. Matthew Forgit on July 1, 2012.
As residents of Crookston, Dr. Miller and her husband are in the process of raising four children. Community involvement is also a source of enjoyment for Dr. Miller and she has been involved in various organizations over the years. Currently, she devotes much of her time to her family and also volunteers on the RiverView Foundation Board, in her church, and for the Villa St. Vincent Advisory Board.
Dr. Angela Smith
Dr. Angie Smith is a native of Stephen, Minnesota and a graduate of Stephen-Argyle High School. In 2002, she graduated from the University of North Dakota with a bachelor’s degree in Nutrition. In 2006, she received her Doctor of Optometry degree from Pacific University College of Optometry in Forest Grove, OR. During optometry school, Dr. Smith traveled to Mexico and Ghana, Africa for eye care mission trips as part of a group called Amigos.
After obtaining her degree, she completed a residency of ocular disease at the Spokane VA Medical Center in Spokane, Washington. After working in Wisconsin for nearly two years, she and her family moved to Crookston in 2009 to work at the Crookston Eye Clinic. In 2012, the Fosston Eye Clinic opened and her patient care time is spent at both locations.
Dr. Smith is a member of the American Optometric Association and the Minnesota Optometric Association. Locally she is a member of the Crookston Noon-Day Lions and Trinity Lutheran Church. She serves on committees at Trinity Lutheran Church and Cathedral School. She and her husband, Nathan, enjoy living in Crookston with their four children: Isabelle, Lucy, Beau and Penelope.
